The requirements checklist is for you to use in preparing your local Reflections Program entries for submission to the Montana PTA for state judging.  Please keep this list for your records.  If the local entry packet is not complete, it will be disqualified.  Use this requirements list for submitting works to Montana PTA.  Some of the requirements are different when sending entries to National PTA such as Literature. 

Forms (3 total):

1.      National PTA Official Student Entry Form completely filled out for each entry.

·        National PTA Unit ID Number can be found on the Unit Leadership Information CD under the “General PTA Information” tab.

·        Original signatures of students and parent in INK! (Important)

·        Each student/participant is required to submit an artist statement to describe how his or her piece relates to the theme.  Make sure each participant has completely filled out the Official Student Entry Form.

·        Reflections Chair information filled in on the Official Student Entry Form.

·        Confirm that your PTA is in good standing.  Check with your PTA Treasurer.

2.      Montana PTA Reflections STATE - Unit Entrants Submission List completely filled out.

3.      Local PTA Participation Form filled out completely.

·        Include a check for $15.00 made payable to the Montana PTA for return of artwork.

Literature:

o       Send four copies of each entry. Remember to keep one for the child as literature entries are not sent back.

o       Write the name of the student on the back of each entry.

o       Place 3 copies and original entry in separate file folders.

o       Make three copies of the Official Student Entry Form.  Place each copy plus the original entry form inside plastic protective covers and tape to backside of each folder.  (Please put the original entry and original entry form together.) 

Music:

o       Rewind tape if necessary.

o       Pack each entry in a large envelope.

o       Place Official Student Entry Form inside plastic protective cover and secure to back of the envelope.

o       Write playing time on each envelope. 

Visual Arts:

o       Entries must be properly mounted (please see specific rules for guidelines).  Matting and shrink wrapping (not lamination) is highly recommended. 

o       Place Official Student Entry Form inside plastic protective cover and secure to the back of the entry. 

o       Dimensions of the piece and description of the medium is required information on the Official Student Entry Form. 

Photography:

o       You may consider submitting enlarged photographs.

o       Photo is properly mounted on foam or matt board.  They do not need to be shrink wrapped.

o       Place Official Entry Form in plastic protective cover and secure to back of entry.

 Film/Video Production  and Dance Choreography:

o       Submit the videotape, CD or DVD in a plastic or cardboard container; identify both the videotape/CD/DVD and container with the student’s name, grade division, state, and year.

 

o       Pack the videotape, CD or DVD in a large envelope.

 

o       Secure the Official Student Entry Form to the outside of the envelope with masking tape.

 Sending Entries:

o       Enclose completed STATE – Unit Entrants Submission List and Local PTA Participation Form

o       Enclose a check for $15.00 made payable to the Montana PTA (no stamps or metered tapes) for return of artwork.

o       Enclose a large filled out shipping/mailing return label.

o       Send all entries and forms to:

Thank you for taking the time to complete each step, they are all important!


 

  • Prepare a report for your PTA and for your Reflections project folder for the next year's chairman.

  • State winners will be notified and announced in the Montana PTA Bulletin.  Judges awards and copies of entries sent to National will be displayed for the public at convention.  Those entries will be returned to you at convention.  All other entries will be returned to the locals along with the participation ribbons and judges award ribbons.  Specific instructions on how to arrange return shipping will be included in the second mailing, in January.
